guess ill tell one on myself. Back in highschool we had a piece of tin come loose on our old barn. Some me and the neighbor kid stuck an extension ladder in the bed of the truck and up I went with my little mikatia cordless drill to put in some screws. well the ladder was a lil short and instead of climbing down and raising it another notch we had the bright idea to have him pull the rope to raise the ladder with me on it. (I was like 125 pounds and he was a big boy) I pulled my self up as he pulled the rope and alls well until we realize it did not lock in the next rung but went into retract mode...fast. I came down in the truck bed and landed on my back and the drill hit him square on the head. Only injury was to our pride!
