This sort of failure rate isn't a generic sludge problem, but a specific one. As in the sludge building up somewhere other then just between the rockers in the head. Nissan has a problem with this on their variable timing camshafts. The orifice to the controller is low, and does get blocked with sludge as a result. This component promptly fails from lack of oil. Sounds like the Toyota Siena has a similar problem. General sludge buildup does happen, especially with longer intervals between oil changes. I've played with enough automobile and motorcycle engines to keep my oil change intervals for automobiles at 3k. When engines are kept on that schedule, they tend not to sludge, even after many hundreds of thousands of miles. Engines with 5k oil changes, those tend to show some sludging by a mere hundred thousand miles.
