I think that shield tube on the PTO shaft has prevented more major injuries than any other innovation.

That said, farm equipment is still incredibly dangerous. Some things that are still just as dangerous as they ever were:

Grain augers.
Combines in general and corn pickers in particular.

Another thing that makes farming especially dangerous is that farmers often work alone in remote areas. An accident that would be serious but not fatal in an urban area is often fatal on the farm.
