Cat. D4D water in the oil

I am told a Cat d4d has wet sleeves with o rings installed between the sleeves and lower block that can fail from overheating, age, or both, allowing water or coolent to get in the crank case. Coolent was thought to be entering the engine around the injector bosses that thread into the head that also have o ring seals, hence the head was completely reworked and pressure tested at a reputable machine shop and given a clean bill of health. The head was reinstalled to cat. specs., the engine fired right up and ran quite well but upon further inspection the dip stick appeared to have been removed from a milk shake. Any and all information or suggestions regarding this condition will be greatly appreciated. Thank You.
