I custom ordered my 04 powerstroke, but the block heater came standard. I just got off the phone with the head service tech at the dealership and he said if you are going to park it for a period of overnight or longer and its below 32 degrees, then plug it in. It is located behind the right tow hook. If you take the front LP bracket off (its hell if you don't, you will cut up your hand on the auxillary transmission cooler), then you can reach it easily and poke it out through the tow hook opening (there's plenty of slack in the cord to do this). I then used a zip tie and tied the cord to the tow hook so it would stay easily accessible. Problem solved.
