Jim, I can't find a parts blow-up online for a W4, but if it's like the same function on the H, the lockout is simply a 3/8" bolt. It should be easy to spot. It's smaller than those around the edge that hold the transmission cover down. On the H, it was just forward and a little to the left of the shifter and passes straight down through the cover.
Its function was very simply to block the forward motion of the shifter rail. To enable 5th, a 3/8" NC bolt 3/4" long was used as a simple plug for the hole. To block fifth, a longer bolt of the same thread was used (the parts listing unfortunately doesn't say how long!), that protruded far enough down to physically block the shifter rail and fork from sliding the fifth gear into mesh. Elegantly simple.
Trick is, if the W4 was the same rig, there's not much to go wrong with it or break. I suppose if it originally had the longer bolt in it and someone backed it out enough to free up the 5th gear but left it in place, just backed out some, that it could over time vibrate back down to where it was blocking things. See if you can find it and pull it all the way out. If it's only 3/4" long, it's not your problem, put it right back, all the way in. If it's an inch long or more replace it with one 3/4" long and see if it will shift. If you still can't shift into fifth, you've got wear or other issues with your shifter.
(N.B. As I said, this if for an H. There may be enough difference in the geometry between an H and a W4 that the correct length for the NON-blocking bolt is other than 3/4". If you've got a parts book, or someone here does and can look it up for you, it'll be listed and illustrated with the transmission and gears.)
