Can you remind us of the make and model? Some are not what I'd call prone to them, but can develop failures in the linkage that would produce a problem like you describe.
It could (more common than linkage failures) be the cover. (Is that what you're calling the cone? The cover/housing for the pressure plate and the clutch fingers?). Could be that somebody monkeyed with the adjustment of it so that it wouldn't work well with a new friction disc. More commonly, it could be that the pins that the fingers hinge on are worn and not capable of pulling teh pressure plate away sufficiently t9o release the friction disc.
If a graphite block type throwout bearing, excessive wear there could be making for your problem.
