We spent a few years using a gooseneck adapter to pull our 5th wheel since the B&W hitch and gooseneck flatbwed came long before the 5th wheel did.That set up was just OK, but the Companion is much nicer. I decided that when I got a bigger 5th I would get a Companion, which I did. It is the "real deal", with urethane bushings and a locking latch like a semi truck has. Its quiet with no clunking when towing. I used mine with a plastic bed liner and I know it probably presses the liner down and rubs the bed some but thats ok. You are supposed to draw it down against the latchin pin and then tightn the u-bolts to hold it tight against the bed so it doesn't rattle. Its not cheap and I think it is the best setup.
