I looked at a lot of 5th wheel hitches when I bought our 37 ft trailer back in 2009. I decided on the B&W turnover ball with Companion, and am very happy that I did so. It is one of the Cadillacs of hitches.
But do follow the instructions and torque the tension bolt and the U bolts down as instructed. IIRC this is spec'ed to be 75 ft lbs on 1/2" bolts. A couple years ago, there was a problem with a trailer that got loose (must have been some real bad road?), and B&W confirmed the absolute necessity of torqueing it down. This is a critical step for safety!
My F350 has a plastic bed liner, so it is slippery, but the hitch is rock solid. I also pull a big GN flatbed and occasionally a GN horse trailer. So the B&W system was the correct answer. I find the rules (laws) strange in that you need to have safety chains with the GN trailer, and no safety chains with a 5th wheel hitch. The 5th wheel hitch seems to me to be more likely to lose a trailer than the GN does.
