It's a concern on all but the common-rail engines. The fuel lubes the pump on the 1st-gens, 89-93, and if it's not supplying enough fuel, burns up the pump.
You can run 2-stroke oil in your fuel to help lube the pump. I'd stay away from the additives that claim better mileage, they don't do much for you.
When I started running 2-stroke oil in my 90 I got another day of driving to work out of a tank of fuel, and it seems to be quieter now, too. I run about a quart per 50 gallons, that's 200:1.
If you're worried about it, change it. It can't hurt anything, and I don't think they are all that much coin.
I don't know about the second-gen 12 valves, you might be able to find some info at the diesel truck resource forums. You'll want the section on second-gen (94-02), 12 valve.
