I had this happen (same symtoms anyway) turns out one of the forks on the pressure plate turned in and soo when I hit the pedal I only was able to push in 2 of the forks not hitting the third one at all. I was about to pull the clutch. and then found this and it was it.. i dont know its not the first thing I though of and aperatly its not common but look to see if all the forks are at the same distance and that the throw out bearing hits them.
