This is all great information. I am currently restoring an MD with the dual range transmission. It has a lever on the left side of the transmission. I am having to measure bearings and try to find since Case IH doesn't have any parts. Installing this aftermarket option wasn't an easy task as both shafts in the transmission had to be modified or replaced in addition to cutting the hole in the side of the transmission housing. That was a good point about the PTO speed as I never thought of that.
