I have seen where the bottom of the lifter wore through allowing the push rod to fall and ride on the cam lobe. This caused the push rod to get free of the exhaust rocker arm but it was still getting an intake charge and firing that cylinder. The exhaust rocker was still getting hit by the push rod so some exhaust was escaping that way. When the intake valve opened it the rest of the exhaust escaped through it first causing the small thumping sound if I remember correctly.
