Rob, from what you describe it sounds like your throwout bearing has had it. If you remove the inspection cover on bottom of clutch housing you should be able to see what is going on in that area. Don't be surprised to see a lot of pieces fall out when you remove the cover. Sounds like one or more of the release levers may be worn quite badly on the pressure plate consequently not releasing the clutch properly. Clutch time.
