Rob, sorry to hear that. It does appear from that stand point that the TA is heading out. There are many nice features with a TA, but being hydraulically operated and part mechanical, many variables enter into the picture. A lot of what you stated at the beginning led me to believe it was either one finger completely worn off and not releasing or the throwout bearing cocking and not pushing squarely on the release levers.
