Also, check the bottom of the lever where it rubs on the plate.Mine wore a groove there, which allowed the lever to twist itself out of the notch. I filled the groove with weld and ground it smooth. I also welded up the pivot hole in the lever and drilled it out to the original size, and replaced the pivot pin, which was worn halfway through.
