Bus Driver: That is the way I see it also. I've had 6 of these in my time and have had this happen more than once. It's just the levers out of time with rockshaft. I find it usually happens by the frame around the levers getting bump and moving out of position. I have found also, it's usually the lever you are not using at the time causing the problem. Quite and easy fix really, and yes not in the manual either IH operators or IT service.
