Farmall H pistons and sleeves

I had overhauled my friends H and he has maybe 30 hours on rebuild. (Guess hrs) Anyways, it ran very smooth and all at once it made a bang. So I took off the pan and a sleeve had totally come apart and was laying in the pan in pieces. Also, the skirt was broke off the piston. A local machine shop had installed the sleeves so I would think they would have checked the bores for out of roundness. Has anyone had the same problem? Could it have been a defective sleeve? Any opinions would be appreciated. Thanks
