Here is another email I sent to Chad.

If you send me a SASE, I will send you a rubbing (like a gravestone
rubbing) and include some dimensions. There are a total of nine holes.
One hole is blind and takes right angle to empty out the side. Thre are
four different hole sizes plus one threaded female NPT. It could be made in
a drill prress.
