ATW: Your problem is the shifting rail for 1st and reverse. Those will have detent balls to hold in position, probably those balls and the shift rail are getting worn. I rather doubt the end of your shift lever is worn, or it would have gone over for 1st and reverse gear. That rail can't move very far before 1st or reverse would be grinding or engaged. Had 1st or reverse been engaged you certainly would not have used the other gears.
