My three decades of dealing with these bearings (and other pilot systems) is that alignment has the biggest effect, followed by compression loading (axial load forcing the parts down the shaft). The last item is lubrication loss from heat, or none from oil source as designed. Look at the failure to see what the dammage is. if the coupling system is dammaged there could be wildly excessive load in this area. Just some ideas.
