Super H Stuck in gear help

My Super H (SN# 9791) is stuck in 3rd gear.
We tried getting it out of gear, but we had to
take the transmission plate off. We replaced
all 4 of the springs and the ball bearings on
the forks that moves gears. We did not put the
plate back on, but put the forks back in place
and tried to shift it with the lever. 1 & R and 4
& 5 shift fine. It still will not shift out of 3rd.
Could it be the wear on the shifting fork or the
hole the ball bearing sits in? What could be
the problem? Where can I get a new fork for
2nd/3rd gear if I need one? My local parts guy
couldn't get them. Thanks!
