Could be anything! Worn parts? I had fix 2 in the past few years first one was an M Jumped out of 5th gear. Shifter fork was fine so took off the platform (seat, gas tank, pulley, steering post, and other stuff) Shfting rails were good and the ball and detent spring was the problem, well the spring broke. Second was a Super M jumped out af 5th and took apart and nothing broke (ball-detent spring), Nothing worn out but their was lots of gunk in their, The oil over the years built up a wax like substance and coated things. took many hours and lots of cleaner and problem solved! Both have not had problems since!
