When I needed one, I went to the local NAPA autoparts store. He went to his bolt cabinet and pulled out one that would work. Did not have the hole for the cotter pin or a fine thread castle nut, but those are not really needed anyway. If you want to put a cotter pin in for security of the nut, just drill a hole for it.
