Yep, worn shifter, stuck in two gears. I would bet a million dollars that your clutch and TA linkages are also out of adjustment, which makes shifting hard and causes the worn shifter to act up. Get yourself an I&T manual and adjust the clutch/TA linkage EXACTLY as the book shows. Weld up the end of the shifter and grind to shape. Will shift like a new one.
