The main cause of failure is age, simple wear and tear. Most of the mechanical TA tractors are pushing 50 years old, on their original or second TAs, and have tens of thousands of hours on them. They're just plum wore out! Improper usage and lack of regular adjustment will shorten a TA's life, and if it's really out of adjustment or someone yanks back on the lever going downhill to hold a load back... Yeah you can really do some damage. Both the owner's manual and the I&T service manual have very precise adjustment procedures in them for TA and clutch adjustment. Regular adjustment, and using the TA properly will result in many years of service. If you replace the TA in your typical collector/show/hobby-farm tractor, you'll NEVER use it enough to wear the TA out in your lifetime.
