It sounds like you should have put the
head back on before working the crankshaft
nut. That way the sleeve wouldn't have popped
out.

You shouldn't have to remove the engine or even
the crankshaft for this job, just take out
the piston and sleeve in question. To
get the sleeve off, I would just do some
careful grinding but beating the piston out
does work as well.
