The answers in order: The driven gears in the transmission are sliding into mesh with the drive gears. Pulling the lever into a gear position with the clutch pedal up, causes those teeth to smash into each other (not good) they resist this smashing with noise and little pieces of them are splintered off into the oil. Do not do this. Fifth gear is a direct coupling of the input shaft to the output shaft. (no gears involved in the coupling, just splines on the 4th fifth slider. It is worn out from the above smashing into gear. It ill not work until the slider and input shaft and intermediate pilot bearing are replaced. The PTO may be gummed up on the inside or the shifter for it is not working. If you can turn it with a pipe wrench while the engine is off and the tractor is in gear, it is probably gust gummed up.
