Google: Create a fillable form and you should get a link to MS Support instructions... BUT they might be referring to form that can filled-out, then submitted ONLINE. Not sure.
Says you need to show the Developer tab: File Options Customize Ribbon - in the list under Main Tabs, select the Developer check box, and click OK.
Then it gives more instructions as to how to use the Developer tab's tools to do what I think you wish to do. Also mentions Building Blocks
