Before pouring the floor, you install a special type of plastic pipe (Wirsbo, other brands, etc.) over 2 inch styrofoam panels. In our house basement we tied the pipe to cattle panels laid on top of the styrofoam- now there are fasteners available that just push into the styro. Pipe is flexible but stays put when bent, is laid about every 12-15 inches in the floor. Water temp is not excessive- just needs to be somewhat above, but less than double that of the desired room temp. Water is not hot enough to cause distortion. You don't get rapid room temp changes.
