Around here the well is required to be sealed from about 3' down to 15' with grout, and the slab is not connect to the well seal. The space below the grout seal to the water table should be sealed with clay or bentionite. That doesn't mean they area all done that way. Does yours have a galvanized metal caseing, if so they do fail after a few decades. I have seen new plastic caseing wells which was never sealed at all. Also could there be another unpluged (burried) well close by? The worst thing about the inflow water is it may have bacteria in it. Another place I found a open drilled hole, tool lost in it, covered by plywood about 2' down, every time it rained our well mudded up.
