My F250 was doing something like that and it had several things wrong, first those fuel and return lines have a rubber sleeve under all of those brass nuts and even if they have never been off they go bad. Second was the rubber hose that connects the steel line from the tank to the transfer pump was cracked.Third the rubber hose I used to bypass the water separator was cracked.And I have also seen a hole rubbed in the side of the steel line from the tank along the frame by the exhaust manifold. I had also replaced all those rubber return lines between the injectors and orings under the tees also.
