484 Clutch Problem

We have a 484 IH that we bought new in 85, the first clutch went out in 3-96 at 1300 hrs.,second clutch in 1-01 at 2050 hrs., and now at 2300 hrs.This tractor does very little loader work and was used with a round baler for only a couple years. In 1-01 installed a rebuilt clutch and pressure plate and had flywheel resurfaced. The last two pressure plates had one of the fingers worn off while the others were barely worn.Also the throwout bearing was in pieces.Could someone explain why this keeps happening? Thanks for any and all help.
