Worn valve guides/valve seals will usually cause an engine to smoke only for a few seconds after start-up and will usually not let enough oil into the combustion chamber to result in fouled plugs. Sorry to say it but I think the 'ol gal is in need of a total rebuild-- sleeves/pistons and rework the head. You might get by with a re-ring job and grind the valves, but that would be a short-term, band-aid solution, and you will be doing it over again in a couple of years or less.
