Doug - An H cannot "come without 5th gear".
An H can be shipped with 5th locked out with the special bolt in the deck plate. However unless 4th gear is also missing (ie. the 4th/5th sliding gear omitted on the mainshaft) there's no way 5th can be left out.
More likely either the 4th/5th shifter rail is not sliding far enough forward to engage 5th. Or (a long shot) possibly is many hours of careless operation have worn the teeth from the 5th gear sliding clutch.
