OT- Help with over 54,000 lb plates

This is my new grain mover, well this and a trailer I haven't got yet. 
What I need to know is what I have to do to get plates for her. 78+ farm plates is what I need but I have been told I need to fill out a form from the local IRS office before getting the tag. So does anyone know what form and what the cost will be? I have 25,000 bushels to move this coming month and 15,000 the following and that is if I don't sell anymore. Plus I would like to move some sand and limestone so the sooner the better on the plates. Thanks in advance!
