front rim slipping

I just bought a new front tire and tube for my H. I remounted the one piece rim using the three clamps. I had a heck of a time getting those nuts tight enough! After plowing some snow I inspected the tire only to find my new valve stem cut by the clamps from the rim slipping around. Looking at the back side of the casting, there was too much clearance to keep the hex head bolts from turning while tightening up the nut. I figured that square head bolts must go there. But book shows hex head. I also searched the archives and others have had problems too. The only suggestion I could find was to use lock washers on casting side and rim side. Does anyone have any other suggestions for keeping the rim tight and in place? Thanks!
