Model C Power Loss- Dies Under Load

My 1948 Model C has been running well for several years. End of last season, the engine began to miss, lacks engine speed, wants to see more choke, even when hot, and bog down under load. Noticed it also died more going up hill, but that again is increased load. Have rebuilt carburetor, changed fuel filter with no change. Changed spark plugs, no change. Noticed water leak in radiator. Noticed antifreeze around head gasket on spark plug side of engine. Any recommendations for next step?
